At Vita Nova, you'll have the opportunity to make a meaningful impact while working in a supportive and collaborative environment grounded in trauma-informed care, positive youth development, and the belief that every young adult deserves the opportunity to thrive.  Position Summary:Vita Nova is seeking a compassionate and resourceful Housing Case Manager to support implement and administer its Permanent Supportive Housing (PSH) program for young adults with documented disabilities who are experiencing homelessness.
