We are currently seeking a Case Aide (Human Service Assistant 2) to join our Aging and People with Disabilities team based in either our John Day or Burns office and providing support to both locations.

  • John Day and Burns are rural communities in Eastern Oregon. If you are considering relocating and are unfamiliar with the area, we encourage you to explore local housing options and learn more about the communities.

  • The selected candidate may choose the office closest to their residence as their base location.

  • The schedule for this position will include designated days assigned to each branch, with regular onsite work supporting both the John Day and Burns offices.

Summary of Duties

As a Case Aide you will:

  • Travel regularly between the John Day and Burns APD offices, following a set schedule with designated onsite workdays at each location.

  • Maintain and update electronic case files, ensuring required documents are obtained, organized, and narrated daily.

  • Prepare client packets, forms, brochures, and other necessary documentation.

  • Contact clients to schedule assessments, appointments, and interpreters; manage Case Manager's scheduling as needed.

  • Support case managers provide long-term care to seniors and people with disabilities.

  • Conduct home visits for signatures, wellness checks, form completion, and delivery of essential items.

  • Assist clients with placement visits, move coordination, and securing household items through community resources.

  • Obtain bids and coordinate referrals for KPlan ancillary services and other specialized supports.

  • Monitor client safety and functioning, reporting significant changes or concerns promptly to Case Managers or Supervisors.

  • Coordinate community service referrals such as RN contracts, Lifeline, Meals on Wheels, and manage facility alerts.

  • Provide administrative and team support, including occasional front desk coverage and issuing EBT cards.

  • Be a contact for Home Care Workers about vouchers, payments, and general questions.

  • Manage mail, upload documents, schedule appointments, and handle calendars and office communication.

  • Keep case managers informed, document actions in case files, and adapt to their differing work styles to support team tasks, meetings, and client needs.

Minimum Qualifications

  • A valid driver's license and acceptable driving record are required for this position. AND

  • One year of experience assisting professionals in one or more of the following specialized social program areas.

    • Economically disadvantaged

    • Substance use and dependency

    • Disability Services

    • Child abuse and neglect

    • Juvenile delinquency

  • Any degree in a related mental health or human services program may substitute for six months of the required experience.

  • Experience communicating respectfully with people from different cultures, whether in person, in writing, or over the phone.

Working Conditions

  • Travel between John Day and Burns APD offices on a regular basis.

  • Work will take place in offices, participants' homes, and various community sites.

  • Travel is required for conducting home visits, completing other case-related tasks, and for attending meetings or trainings as needed.

  • Rare overnight travel may be required.

  • You will work a professional work week of 40 hours per week, with possible fluctuations based on service needs.

  • There may be rare, optional compensated overtime, including evenings and weekends.

  • You may work with individuals experiencing trauma or crisis, some who may have difficulty managing their emotions.

  • Quick decision-making may be necessary to ensure the safety of yourself and others.
