Job Description Summary

Millennium Physician Group is seeking to hire a full-time / Case Manager to join the ACO team. We are looking for a new team member who is positive, energetic, well-organized, can multi-task, and think outside of the box. We want someone who supports our top initiative of ensuring an excellent patient experience.

How will you make an impact & Requirements

Hours Monday-Friday 8am-5pm-Full-Time

This is a traveling role- 3 days a week to offices in Melbourne/Vero Beach

2 days will be working from home (schedule can vary)

The Case Manager is responsible for the direct provision of services that assist patients with the achievement of their highest level of wellness. The Case Manager focuses on client-centered services that link clients and their family members with health care, and other home or community-based services to ensure timely, coordinated access to medically appropriate levels of health and support services and continuity of care. They are responsible for assessment, planning, coordination, advocacy, education, monitoring, and accurate, timely documentation in all program areas. They demonstrate knowledge of growth and development and possess the ability to assess, interact, communicate, and care for age-specific patient populations.

Required Knowledge and Experience

Registered Nurse; active and clear license in the State of Florida. The license must be maintained once employed. 3+ years of Clinical experience with 1+ years of case management experience preferred.

Required Skills and Abilities

To perform this job successfully, an individual should have strong computer skills, especially in Excel, and knowledge of EMR systems; Athena knowledge preferred but not required.
