Job description

JOB PURPOSE:

The Lead Case Manager will have general oversight and coordination of other case management. The Lead Case Manager ensures staff are completing intake, coordinated assessment, individualize housing plan, and making appropriate community referrals for additional services. The Lead Case Manager ensures staff maintains appropriate contact with the client, that quality services are provided and that the individualize housing plan is completed timely.

ESSENTIAL JOB RESPONSIBILITIES:

· Oversee all services and operational activities.

· Provide comprehensive case management that includes, but not limited to: screening and assessing clients; creating, implementing and monitoring individual service plans; crisis intervention; connecting clients to health, mental health, employment, housing and other community resources.

· Foster a professional and welcoming environment in all interactions with clients, guest, vendors and colleagues, regardless of differing identities, backgrounds, beliefs and choices.

· Manage and evaluate the performance of assigned staffed.

· Develop and maintain a professional and supportive relationships with staff and subordinates.

· Provide information regarding services and transportation as needed to people seeking services.

· Work in tandem with case managers to assist clients in reaching their case plan goals.

· Continually monitor and evaluate each client’s progression through their case management plan, and develop corrective action revisions to the plan as needed.

· Maintain accurate documentation of service objectives and outcomes as well as other services in accordance with established guidelines.

· Plans, coordinates, and/or facilitates life management classes to assist clients in obtaining skills that will enable them to functionally cope with their environment; obtains speakers and workshop presenters as needed.

· Meet all federal and state regulatory guidelines and standards that are applicable to this position.

· Participate in workshops, seminars, education programs and other activities that promote professional growth and development.

· Compile, prepare, submit and maintain accurate records, files, forms, statistics and additional information in accordance with agency policies, licensing and/or contract requirements.

· Participate in weekly case staffing with case managers.

· Maintain confidentiality in all areas of the service population and program operations.

· Other duties as assigned

ESSENTIAL QUALIFICATIONS:

EDUCATION: Bachelor’s Degree in Social Work, Sociology, Psychology or related field preferred.

EXPERIENCE: Three years case management experience working with homeless, low

income, veterans & their families is a strong plus. Supervisory experience leading other case managers is preferred. Demonstrated ability as a servant leader is preferred.

ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer’s sole discretion.

LICENSES: Driver’s License with a clear record.

VEHICLE: Must have daily use of a vehicle without prior notice.

OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates. Must not pose a direct threat or significant risk of substantial harm to the safety or health of himself/herself or others.
