Hospice Registered Nurse Case Manager

Location: Inland Empire, CA/Riverside and Surrounding areas

Lead with Compassion & Excellence in Hospice Care

Are you an experienced Hospice RN who thrives in leadership and is passionate about providing high-quality end-of-life care? We are seeking a Hospice Registered Nurse Case Manager to oversee patient care, mentor interdisciplinary teams, and ensure compliance with clinical best practices. This role is ideal for a skilled leader who excels in case management, team coordination, and regulatory compliance, all while delivering compassionate care to patients and families.

Key Responsibilities

Leadership & Team Oversight - Supervise, mentor, and ensure the competency of your assigned hospice care team, maintaining high standards of care.

Patient & Family Coordination - Serve as the team leader, providing direction and oversight to interdisciplinary team members.

Case Management - Oversee care plans, ensuring accurate evaluations, appropriate treatments, and optimal patient outcomes.

Clinical Oversight - Monitor and review patient care, ensuring compliance with Medicare and hospice regulatory standards.

Documentation & Compliance - Review and approve care plans within the Electronic Medical Records (EMR) system, ensuring accuracy and adherence to Medicare Conditions of Participation.

Quality Improvement - Participate in agency quality assurance initiatives, reviewing outcome data and driving performance improvements.

Interdisciplinary Team Meetings - Lead and coordinate team meetings with hospice clinicians and the medical director to ensure exceptional patient care.

Education & Mentorship - Provide ongoing training, guidance, and documentation support for hospice team members, assisting with orientation and performance feedback.

Regulatory Readiness - Assist the Director of Clinical Services (DCS) in maintaining compliance and survey readiness.

On-Call Leadership - Participate in an on-call rotation, ensuring 24/7 oversight of patient care and agency operations.

Qualifications & RequirementsEducation & Experience

Bachelor's degree or higher in a health-related field.

Minimum of 3 years of clinical RN experience, with hospice experience required.

Strong knowledge of Medicare Conditions of Participation, Local Coverage Determinations, and regulatory standards. JCAHO knowledge preferred.

Proficiency with Outlook, Word, and Excel.

Licenses & Certifications

Valid RN license in the state of practice.

Certified Hospice and Palliative Care Nurse certification preferred.

Active BLS certification through the American Heart Association or Red Cross.

Valid driver's license and auto liability insurance for home visits.
