Are you a seasoned healthcare professional with a passion for case management and a proven track record of leadership in acute care settings? Join our dynamic team as the Director of Case Management and play a pivotal role in providing top-notch patient care and ensuring seamless transitions across our 300-bed acute care facility.


Responsibilities:


As the Director of Case Management, you will lead and oversee our discharge team to ensure efficient and effective patient transitions and care coordination. Your responsibilities will include:

  • Providing strategic direction and leadership to the case management department.
  • Collaborating with interdisciplinary teams to develop and implement patient-centered care plans.
  • Managing utilization review processes and ensuring compliance with regulatory standards.
  • Identifying and implementing process improvements to enhance patient care and outcomes.
  • Mentoring and developing case management staff to achieve excellence in their roles.
  • Utilizing data and analytics to drive performance improvements and cost-effective care.

Requirements:

  • Bachelor's degree and graduate of a Bachelor's Program in Nursing.
  • Professional Registered Nursing licensure in the state of Georgia (or compact license).
  • Minimum of five (5) years of experience as a Director of Case Management in a short-term acute care hospital setting.
  • Proven leadership skills and ability to manage and develop teams.
  • Strong understanding of utilization review, care coordination, and discharge planning.
  • Excellent communication, interpersonal, and organizational skills.
