Overview
HOPE Community Services, Inc. is a leading outpatient treatment provider in south Oklahoma City. We provide compassionate trauma-informed care to individuals whose lives have been impacted by mental health and substance use disorders. We do this by providing compassionate and evidence-based treatment, aimed at improving the quality of life for those we serve.
We are currently seeking a dedicated and compassionate Community Support Case Manager to join our team. The ideal candidate will be skilled in case management, advocacy, and resource coordination to help clients thrive in the community. About the Position
As a Community Support Case Manager, you will play a vital role in providing case management services to clients with mental health and/or substance use challenges. You will assist clients in accessing community resources, developing independent living and social skills, and creating individualized recovery plans. In addition, you will be responsible for delivering both individual and group services to help clients build the skills needed for successful community integration. This position requires strong communication skills, a commitment to client advocacy, and the ability to manage multiple responsibilities in a dynamic environment.
About the Program
The Community Support team at HOPE is dedicated to helping clients develop the skills they need for independent living and community integration. By coordinating resources and providing advocacy, the team plays a key role in empowering clients to achieve their personal goals. The program focuses on building supportive relationships and providing trauma-informed care in a collaborative, client-centered environment.
Requirements
  • Ability to obtain and maintain Case Management II (CM II) certification.
