Job description

As a Registered Nurse Case Manager for Carter Healthcare, you are the direct point of contact for patient care and information. You will be heavily involved in supervising the education of patients and caregivers, coordinating interdisciplinary clinical services with your team, and assuring continuity of high-quality care.
