Job description

Role Overview: AmeriHealth Caritas New Hampshire (ACNH) is seeking a passionate and community-driven Community Educator to join our outreach team and help connect vulnerable and diverse populations across the state to essential healthcare resources. Based out of our Manchester Wellness Center and traveling statewide, this role promotes AmeriHealth Caritas programs through community events and site visits, identifies and guides potential Medicaid and uninsured individuals to appropriate services, and educates them on enrollment processes. The Community Educator also coordinates outreach activities, supports event planning, and assists members by directing concerns to the appropriate departments, all while serving as a trusted and compassionate representative of our mission to improve health outcomes in New Hampshire (NH).

Work Arrangement:

  • This is a hybrid role offering flexibility. The candidate is expected to work on-site at least one day per week at our Manchester, NH office.
  • The position also involves travel across the state to support tabling and outreach events.
  • Occasional evening and weekend hours may be required, and the candidate can flex time as needed.

Responsibilities:

  • Educates potential members about our programs and benefits at community events and outreach sites.
  • Plans, coordinates, and manages community outreach activities, ensuring proper staffing and materials.
  • Guides potential Medicaid recipients through the enrollment process and broker system.
  • Assists members with concerns, referring them to Member Services for support.
  • Builds and maintains relationships with community agencies to strengthen local engagement.
  • Supports member retention by helping auto-enrolled members select primary care providers and understand their benefits.
  • Represents the organization professionally at scheduled sites and events, maintaining a clean and organized setup.
  • Adapts to a flexible schedule, including evenings, weekends, and special events.
  • Proactively identifies new outreach opportunities within the community.
  • Monitors market trends and competitor activities, reporting management insights.
  • Accurately completes and submits required documentation, logs, and encounter forms promptly.
  • Facilitates face-to-face education sessions and workshops for members and community groups.
  • Conducts orientations and presentations about AmeriHealth Caritas programs to various audiences.

Education & Experience:

  • High School diploma or GED required
  • Associate’s degree preferred
  • 1 to 3 years of experience in sales, customer service, community outreach, or support-related roles
  • Familiarity with the healthcare industry and local marketplace is a plus
  • Bilingual candidates are strongly encouraged to apply
  • Must be able to travel throughout NH for outreach activities

Licensure:

  • Valid NH driver’s license and car insurance required.
