Job Title: Care Navigator

Reports to: Member Experience Manager

We Care Daily Clinics (WCDC) is a warm, welcoming, and modern facility that provides treatment for addiction in a person-centered environment located in Auburn. WCDC seeks to renew hope and purpose for those battling opioid use by removing barriers, providing a holistic approach to recovery, and utilizing evidence-based treatment to restore healthy living. Our top priority is to provide the highest levels of patient-centered care to those we serve.
WCDC is an Opioid Treatment Program (OTP) facility using FDA approved medication paired with behavioral health counseling. Our individual service plans are built around patients in a supportive, safe environment to create a personalized pathway to recovery. In addition to recovery, we also offer planned patient transportation and family and social support.
Position Summary:
The Care Navigator plays a pivotal role in supporting patient care and engagement by providing on-demand peer services, crisis intervention, and de-escalation support. Stationed in the lobby, the Care Navigator serves as a vital resource for patient engagement, pre-registration assistance, and overall care coordination. Acting as a bridge between patients and healthcare providers, the Care Navigator ensures seamless access to services and support throughout the patient's care journey.
Key Responsibilities:
Patient Engagement
  • Assist patients with the Web-App and other pre-registration tasks, ensuring they receive the support needed to navigate their healthcare journey.
Crisis Intervention
  • Provide immediate support for mental health crises, including suicidal or homicidal ideation, by employing motivational interviewing and de-escalation techniques.
Queue Management
  • Monitor patient queues to ensure timely service, alerting the SUD team if a patient has been waiting longer than 5 minutes.
Crisis Support
  • Offer emotional and de-escalation support across departments, helping patients navigate overwhelming feelings and barriers to care.
Administrative Support
  • Manage critical reports, including the 45/90-day report and UA intervention report.
  • Support SUD teams with rooming and patient care as needed.
Departmental Coverage
  • Provide backup support for the PSR desk and the Neighborhood Center.
  • Answer questions related to the SUD/Peer program and assist with overall clinic operations.
Additional Duties:
  • Arrange patient transportation as needed.
  • Generate billable check-ins for peer services.
  • Advocate for patients, helping them overcome barriers to care.
  • Ensure smooth transitions for unenrolled and outreach patients.
Qualifications:
  • Strong interpersonal and communication skills.
  • Experience in crisis intervention and motivational interviewing preferred.
  • Familiarity with healthcare or peer support services is a plus.
