Job description

Care Manager I (Hybrid) 

V-Tech Solutions is seeking a Care Manager I to support children and young adults with special needs by providing comprehensive care coordination services. The role focuses on assessment, care planning, service coordination, and collaboration with families, healthcare providers, and community resources to ensure quality, member-centered care. 

 
Key Responsibilities:

  • Manage assigned caseload 

  • Conduct assessments and community outreach 

  • Develop and monitor individualized care plans 

  • Coordinate medical, behavioral health, and community services 

  • Support discharge planning and transitions of care 

  • Educate members and caregivers 

  • Document care activities accurately 

 
Qualifications: 

  • Master’s Degree in Social Work Required 

  • Active DC LGSW license 

  • Two or more years of care management or community health experience preferred 

  • CCM certification preferred 

  • Spanish preferred
