Care at Home Michigan is a growing Home Help agency dedicated to helping seniors and individuals with disabilities remain safely in their homes. We are seeking an energetic, outgoing, and highly organized professional who enjoys building relationships, coordinating care, and representing the agency in the community.

This is an excellent opportunity for someone who enjoys working with people, solving problems, and making a meaningful impact while helping grow a rapidly expanding healthcare organization.

What You'll Do

Client & Care Coordination

  • Coordinate services for new and existing clients.

  • Communicate regularly with clients, families, caregivers, and case managers.

  • Track authorizations and ensure services are implemented promptly.

  • Resolve client concerns and service issues.

  • Maintain accurate client records and documentation.

Community Relations & Business Development

  • Build and maintain relationships with case managers, social workers, discharge planners, and community organizations.

  • Conduct regular in-person visits to referral sources throughout the community.

  • Attend networking events and provider meetings.

  • Represent Care at Home Michigan professionally in the field.

  • Help grow the agency through strong community partnerships and referral development.

Administrative & Billing Support

  • Assist with Medicaid billing and claim follow-up.

  • Track authorizations and service utilization.

  • Maintain accurate information within agency software systems.

  • Generate reports and support office operations.

  • Assist with scheduling and coordination activities as needed.
