Job description

St. Peters Health Partners is seeking a part time Hospice Physician to join The Community Hospice serving the Capital Region. This is a meaningful opportunity to provide compassionate, patient? centered end of life care while working within a highly respected, mission driven organization in the Capital District.

Position Details

  • Collaborate closely with the interdisciplinary hospice team, including nurses, social workers, and chaplains, to develop and guide individualized care plans
  • Provide physician oversight and medical management for hospice patients across home, skilled nursing facility, and hospital settings
  • Serve as a clinical resource to community medical providers and prescribe medications as appropriate
  • Certify initial and ongoing hospice eligibility and determine the appropriate level of hospice care
  • Participate in interdisciplinary team meetings and care planning conferences
  • Engage in education of medical students, NP/PA students, residents, and Geriatrics Fellows
  • Share on call responsibilities with a team of five hospice physicians

Competitive Compensation

St. Peters Health Partners offers a comprehensive compensation package, including:

  • Compensation of $140,000
  • Starting bonus
  • CME allowance and professional development support
  • The posted salary reflects the starting range of total compensation and does not include productivity bonuses, quality or performance incentives, extra shift incentives, or other forms of cash compensation as applicable. Salary range provided in accordance with State law and may vary based on experience and other factors.
