Position Overview
The Value-Based Care Manager is responsible for coordinating and managing the care of oncology patients participating in value-based care programs. This role serves as a central point of contact for patients and caregivers, ensuring care coordination, reducing duplication of services, and supporting positive clinical and cost-effective outcomes. Working collaboratively with physicians, nurse navigators, and interdisciplinary teams, the Value-Based Care Manager helps patients navigate the healthcare system while ensuring compliance with program requirements and practice redesign initiatives.
Key Responsibilities
  • Serve as a central point of contact for patients at high risk for complications to ensure coordinated, efficient care.
  • Utilize a collaborative care management process including assessment, planning, facilitation, coordination, evaluation, and advocacy.
  • Work closely with the patient's primary care team, particularly nurse navigators, to prevent overlap or redundancy in care.
  • Assess patient, family, and caregiver needs and provide education, anticipatory guidance, and resources to reduce barriers to care.
  • Perform appropriate clinical actions within the scope of nursing practice and training.
  • Educate patients on preventive care practices and reinforce individualized care plans to prevent unnecessary admissions and readmissions.
  • Track patient progress and outcomes; adjust care plans as needed in collaboration with oncologists.
  • Schedule appointments, coordinate with physicians and specialists, and arrange outside services to support patient care and well-being.
  • Assist patients in managing chronic conditions, prioritizing healthcare needs, and preventing complications through standardized care protocols.
  • Support care transitions for patients discharged from the hospital, including medication management and follow-up scheduling.
  • Collaborate with provider clinic staff to ensure accurate documentation of staging, oncology/hematology diagnoses, and other relevant clinical information.

Qualifications
Education & Experience
  • Required:
    • Bachelor's degree in Nursing.
    • Minimum of three (3) years of nursing experience.
  • Preferred:
    • Master's degree.
    • Case management experience.

Credential/Licensure
  • Required:
    • Current RN license to practice in the state where work is performed.

Skills & Abilities
  • Required:
    • Strong knowledge of healthcare management resources and care coordination services.
    • Excellent verbal and written communication skills with strong problem-solving and conflict resolution abilities.
    • Ability to communicate effectively with patients, families, physicians, and staff at all levels.
    • Proficiency in Microsoft applications (Word, Excel) and understanding of patient privacy and security requirements.
    • Ability to work independently, exercise sound judgment, and manage multiple tasks and projects effectively.
    • Strong organizational skills with the ability to maintain control of workflow in a fast-paced environment.
  • Preferred:
    • Experience working within oncology or value-based care programs.
