APRN / Nurse Practitioner – Adult Medicine



This organization participates in the NHSC Loan Repayment Program


This community-based health center has provided comprehensive healthcare services to residents of Hartford and Windsor. Established for the community, by the community, the center offers a full spectrum of services including medical care, behavioral health, dental care, a full-service pharmacy, and more. The mission is to provide patients with a true medical home—a place where all their healthcare needs can be met, where clinical staff know them by name, and where care is delivered with dignity, compassion, and quality.


The Adult Medicine Clinic is seeking an experienced APRN/NP to join the team. This role provides comprehensive primary health care, including health promotion and disease prevention, while collaborating with physicians and referring patients to specialists as appropriate.


Key Qualifications:

  • Graduate of an accredited school of nursing and completion of an approved nurse practitioner program
  • Valid State of Connecticut licensure
  • DEA and Connecticut narcotic registration (or eligibility to obtain)
  • Strong knowledge of nursing or physician assistant theory and practice, with a proven record of delivering high-quality, comprehensive clinical care
  • Ability to provide culturally competent care to a diverse patient population in a collaborative environment
  • Experience taking medical histories, assessing conditions, and interpreting findings
  • Strong communication skills (oral and written), with proven success engaging patients, peers, and other stakeholders
  • Demonstrated ability to maintain thorough and accurate clinical documentation promptly
  • Evidence-based clinical decision-making and adherence to quality measures
  • Comfort with electronic health records and related clinical systems
  • Willingness to perform other duties as assigned
  • 1–2 years of experience in an ambulatory or urgent care setting preferred
