Position Overview (Director of Admissions)

Polaris Rehabilitation & Care Center is seeking an experienced, relationship-focused, and highly organized Director of Admissions to lead our admissions and referral efforts. This leadership position is responsible for overseeing the admissions process, developing strong referral partnerships, coordinating resident admissions, and supporting census growth while ensuring an exceptional experience for residents and their families.

As the Director of Admissions, you will serve as the primary point of contact for prospective residents, families, hospitals, physicians, case managers, and referral partners. You will work collaboratively with the interdisciplinary team to ensure seamless admissions, accurate documentation, effective communication, and a positive transition into our community.

Key Responsibilities
  • Coordinate and oversee all inquiries, referrals, admissions, readmissions, and resident transitions

  • Serve as the primary liaison for prospective residents, families, hospitals, physicians, discharge planners, case managers, and referral sources

  • Guide residents and families through the admission process while providing exceptional customer service and timely communication

  • Facilitate the admissions process, including completion of required paperwork, insurance verification, and coordination with clinical departments

  • Maintain accurate records of inquiries, admissions, discharges, referrals, and census activity

  • Prepare and distribute daily census reports and admission updates to facility leadership

  • Monitor census trends and maintain monthly census reports and other required operational metrics

  • Conduct follow-up visits with newly admitted residents and complete post-admission satisfaction visits to ensure a positive resident experience

  • Coordinate resident room readiness and admission logistics with nursing, housekeeping, dietary, therapy, and other departments

  • Represent Polaris Rehabilitation & Care Center at community events, health fairs, hospital meetings, open houses, and other public relations activities

  • Build and maintain strong relationships with hospitals, physicians, case managers, social workers, assisted living communities, and other referral partners throughout the region

  • Collaborate with facility leadership to develop and implement census growth and marketing strategies

  • Ensure compliance with CMS regulations, HIPAA requirements, admission policies, and applicable federal and Wyoming regulations

  • Participate in Quality Assurance and Performance Improvement (QAPI) initiatives related to admissions, customer satisfaction, and census development

Qualifications
  • High school diploma or equivalent required; Associate's or Bachelor's degree in Healthcare Administration, Business, Marketing, Nursing, or a related field preferred

  • Previous experience in admissions, healthcare marketing, business development, case management, or long-term care preferred

  • Skilled nursing, rehabilitation, hospital, or post-acute care experience strongly preferred

  • Knowledge of Medicare, Medicaid, commercial insurance, and long-term care admission processes preferred

  • Strong customer service, relationship-building, and communication skills

  • Excellent organizational, time management, and multitasking abilities

  • Ability to work collaboratively with interdisciplinary teams and community referral partners

  • Proficiency with electronic medical records (EMR), Microsoft Office, and admissions software

  • Strong attention to detail and commitment to confidentiality and professionalism

  • Valid driver's license with the ability to travel locally for referral development and community outreach as needed
