Job description

Bridgeway Community Health is currently seeking qualifiedQualified Mental Health Professional (QMHP)to join our growing Adult Outpatient and Co-Occurring Mental Health and Addictions treatment teams in both Salem and Woodburn locations! A sign-on bonus of $5,000 will be available to the successful new external hires!! Primary PurposeThe Co-Occurring Therapist provides dual diagnosis enhanced services using stage-of-change specific interventions with individualized outcomes focusing on reducing mental health and substance abuse symptoms. Essential Duties and ResponsibilitiesConducts integrated assessments, including screening for the presence of substance use, problem gambling, mental health conditions, and chronic medical conditions, as well as the presence of systems-related trauma using ASAM criteria and DSM.Utilizes assessment tools or data in the evaluation, treatment planning, and discharge process, including cognitive behavioral interventions, motivational interventions, and healthy relationships education.Prepares and maintains all required treatment records and reports.Counsels clients in individual and group counseling sessions. Individual sessions may periodically include family members or support people.Counsels family members to assist them in understanding, dealing with and supporting clients.Collects information about clients through interviews, observations, and collateral information.Acts as client advocate to coordinate required services or to resolve emergency problems in crises.Develops and implements evidence-based, client-centered treatment plans that address mental health and substance use.Documents relevant treatment reviews, progress notes, status reports, coordination of services, and discharge summaries.Meets with families, probation officers, police, and other interested parties to exchange necessary information during the treatment process.Observes urinalysis screening and completes chain of custody documentation.Gathers information regarding community mental health services and resources to use in conjunction with therapy.Completes discharge paperwork to include the discharge plan completed with the client. Other Duties and Responsibilities Attends staff meetings and collaborates with other team members to identify and implement improvements in the quality of services. Follows new developments in their field by reading professional literature, attending courses and seminars, and establishing and maintaining contact with other social service agencies. Performs other duties as assigned.Minimum Qualifications and ExperienceExperience, Education, and CredentialsRequires QMHP with a graduate degree in psychology, social work, or a behavioral science field from an accredited college or university AND CADC certification. Experience in the treatment of addictions is strongly preferred.Experience must demonstrate the ability to conduct an assessment, including identifying precipitating events, gathering histories of mental and physical health, substance use, past mental health services, and criminal justice contacts; assessing family, cultural, social, and work relationships; conducting a mental status examination; complete a DSM diagnosis; write and supervise the implementation of a service/treatment plan; and provide individual, family or group therapy within the scope of their training.Knowledge, Skills, and AbilitiesExcellent interpersonal communication skills and strong computer and time management skills are required to work effectively.
