Job Description Summary

Providing comprehensive Primary healthcare services and advanced Primary care health services to patients of all ages, including preventive care, diagnosis and treatment of acute and chronic illnesses, and overall health management. The PCP serves as the main point of contact for patient care, coordinating with specialists, managing referrals, educating patients on healthy practices, and maintaining detailed medical records to ensure continuity of care and promote patient well-being.

How will you make an impact & Requirements

Primary Care Physician (PCP) at CareMore Health (now part of Mosaic Health), the role involves providing comprehensive, integrated, and personalized care, particularly for patients with complex or chronic conditions. The position emphasizes value-based care, accountability for patient outcomes, and collaboration within a multidisciplinary team.

Primary duties and responsibilities

  • Comprehensive patient care: Provide care for a panel of patients with multiple co-morbidities, focusing on prevention, diagnosis, and treatment of acute and chronic conditions. Prevention of unnecessary ER visits and Hospitalizations. Identifying high risk members on daily basis and bringing them to discussions during high risk huddles.

  • Multidisciplinary collaboration: Work closely with a team that can include advanced practice providers, case managers, specialists, and others to create and execute patient-centered care plans.

  • Patient education: Educate patients and their families on wellness, prevention, healthy lifestyle choices, and the management of their health conditions.

  • Coordinate referrals: Manage patient referrals to specialists when necessary, ensuring effective communication and coordinated follow-up care.

  • Documentation: Maintain thorough and timely medical records using an electronic medical record (EMR) system, ensuring regulatory compliance.

  • Qualifications and requirements

  • Degree: Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited medical school.

  • Licensure: A valid, unrestricted California Medical Board license, a current Drug Enforcement Agency (DEA) certificate, and potentially a BLS/ACLS certification.

  • Board certification: Board certification or eligibility in Internal Medicine is required.

  • Experience: Proficiency in managing complex and chronic conditions is a key qualification.

  • Skills: Strong diagnostic and clinical skills, excellent communication and interpersonal skills, and a commitment to patient-centered care are essential.

  • Location: Upland, California

**This position is bonus eligible based on individual and company performance.**
