Position SummaryACT Program non profit organization is seeking a compassionate and experienced Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our Assertive Community Treatment (ACT) Team. This role serves as the primary psychiatric provider for adults living with serious mental illness and co-occurring substance use disorders who have historically struggled to engage with traditional outpatient services.
The PMHNP is a vital member of the ACT interdisciplinary team, providing psychiatric evaluations, medication management, crisis intervention, and ongoing psychiatric care in community settings. Services are delivered where clients live, work, and spend their time, including homes, supportive housing programs, shelters, and other community locations.
This position requires a recovery-oriented, trauma-informed clinician who is passionate about helping individuals with schizophrenia, schizoaffective disorder, bipolar disorder, major depressive disorder, and co-occurring substance use disorders achieve stability, independence, and improved quality of life.
As a full member of the ACT team, the PMHNP collaborates closely with therapists, nurses, case managers, peer support specialists, and community partners to ensure integrated, person-centered treatment that supports each client's recovery goals.
Key Responsibilities Psychiatric Assessment & Medication Management- Conduct comprehensive psychiatric evaluations, diagnostic assessments, mental status examinations, and risk assessments.
- Diagnose and treat psychiatric disorders within the scope of practice and Utah prescribing regulations.
- Develop, implement, and manage psychotropic medication treatment plans.
- Monitor medication effectiveness, side effects, adherence, and client preferences using shared decision-making practices.
- Coordinate laboratory monitoring, metabolic screening, and medical referrals as clinically indicated.
- Provide ongoing psychiatric care for individuals with serious mental illness and co-occurring substance use disorders.
- Conduct psychiatric visits in client homes, shelters, supportive housing programs, and community settings.
- Participate in assertive outreach efforts designed to engage individuals who may be resistant to treatment.
- Utilize recovery-oriented, trauma-informed, and person-centered treatment approaches.
- Support continuity of care during psychiatric hospitalizations, emergency department visits, and transitions between levels of care.
- Provide psychiatric consultation and guidance during behavioral health crises.
- Collaborate with emergency departments, inpatient psychiatric units, crisis stabilization programs, and community providers.
- Participate in after-hours and weekend on-call rotation as assigned.
- Provide consultation regarding psychiatric emergencies, medication concerns, and risk assessments.
- Participate in daily ACT team meetings and treatment planning discussions.
- Collaborate closely with nurses regarding medication administration, side-effect monitoring, laboratory review, and client education.
- Contribute psychiatric expertise to integrated treatment plans addressing behavioral health, substance use, housing, medical, vocational, and social needs.
- Support ACT fidelity standards and recovery-focused care delivery.
- Complete timely and accurate documentation in the Electronic Medical Record.
- Maintain compliance with Medicaid, Optum, Salt Lake County, state, and federal regulations.
- Participate in quality improvement initiatives, audits, and ACT fidelity reviews.
- Maintain productivity standards and documentation requirements.
- Master's Degree or Doctorate in Nursing from an accredited program.
- Board Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC).
- Active and unrestricted Advanced Practice Registered Nurse (APRN) license in the State of Utah.
- Full prescriptive authority and DEA registration.
- CPR/BLS Certification.
- Minimum of two (2) years of psychiatric prescribing experience working with adults.
- Valid driver's license and ability to travel throughout the service area.
- Experience working in ACT, community mental health, mobile crisis, homeless outreach, or public-sector behavioral health.
- Experience treating schizophrenia, schizoaffective disorder, bipolar disorder, and other serious mental illnesses.
- Experience working with co-occurring mental health and substance use disorders.
- Experience prescribing and managing long-acting injectable antipsychotics.
- Experience with Medication Assisted Treatment (MAT), including buprenorphine/naloxone treatment.
- Familiarity with ACT model fidelity standards and recovery-oriented care practices.
- Strong understanding of serious mental illness and evidence-based psychiatric treatment.
- Knowledge of ACT Team principles and community-based behavioral health services.
- Ability to engage individuals who may be reluctant or resistant to treatment.
- Strong assessment, diagnostic, psychopharmacology, and crisis intervention skills.
- Excellent interpersonal, communication, and collaboration abilities.
- Ability to work independently in community settings while maintaining strong team communication.
- Commitment to trauma-informed, recovery-oriented, and person-centered care.
