Job description

Shift - Sunday to Thursday 11:30am - 8pm

Schedule - 5 X 8

Contract Length - 13 weeks
Must Have Skills
Familiar with navigating epic.

Background in case management or familiarity with the case manager role.

Must have experience in 1:1 counseling.

Must be comfortable to lead a group in group sessions.

Curriculums are provided - will be expected to lead 1-2 groups.

Must prep for the group meeting, be comfortable redirecting and navigating the conversation if they go off topic.

May be responsible for retrieving medication from the pharmacy - VALID DRIVERS LICENSE REQUIRED.

Rare, but could be asked to transport the patients to locations such as hospitals, urgent cares, train stations, etc.

Two to three years related experience and/or training in the area of chemical dependency.


Job Details:
Orientation will be 1 week or less. MUST HAVE KNOWLEDGE OF CASE MANAGEMENT IN ORDER TO BE CONSIDERED, preferably a background in addictions/recovery.
The Case Manager has the responsibility for providing case management services to individuals who are detoxing from substances. This includes assessing service needs; counseling, developing plans for access to services, liaison with client, families, programs, and service providers.

Knowledge of relevant community resources for continuing care/aftercare placement. Certificates, Licenses, Registrations; Certified Addiction Counselor (CAC) preferred. Must be able to complete on-site CPI and CPR training/certification. Must demonstrate ability to relate to a substance abuse treatment population in a sensitive manner, an interest and willingness to work in a multicultural setting and must share a customer service orientation. Manages difficult or emotional client situations; Responds promptly to client needs. Focuses on solving conflict, not blaming; Maintains confidentiality. Essential Functions/Key Responsibilities: 1. Responsible for maintaining individual case load, which involves conducting individual intake evaluations, treatment and aftercare planning, medication management support, and safety planning as needed. 2. Completes all documentation in a timely manner, as set forth by regulations. 3. Coordinate with collaterals and referral sources as needed. 4. Assist in retrieval of patient medications from local pharmacies. 5. Acts as a support to other staff in milieu management, de-escalation, and crisis management. 6. Coordinate and support transportation needs to necessary appointments and aftercare. 7. Maintain Virtual Gateway access and complete disenrollment assessments as required. 8. Engage in identifying and locating possible contraband on the unit, performing unit searches as needed. 9. Assess patients’ continuing care needs, contacting appropriate aftercare resources and arranging for transfer or admission to appropriate programming. 10. Shares responsibility for education and group programming, including recreation and wellness. 11. Attends staff meetings and supervisory sessions, as scheduled. 12. When possible and/or requested, provide information for affected family members to self-help. 13. Other related duties as assigned. Listens and gets clarification; responds well to questions; demonstrates group presentation skills. Writes clearly and informatively. Ability to respond to common inquiries or complaints from consumers, regulatory agencies, or other service providers. Ability to define problems, collect data, establish facts and draw valid conclusions.
