The RN is responsible for providing professional nursing care for individuals who are supported by the Opportunity Foundation, both residentially and vocationally, and for overseeing all medical service operations within the site they have been assigned to. The RN will assure provision of an excellent health care program within their designated site and to the individuals served there. The RN will be evaluated equally between their skills, leadership abilities and conduct as they apply to the Mission, Vision and Values of the Organization.

Essential Job Functions

  • Regular attendance and punctuality are required.
  • Model leadership qualities to the team that builds morale and confidence while supporting their skills.
  • Assure compliance to all OARโ€™s for licensure as well as the accepted scope of nursing practices, including, but is not limited, assuring medication administration, medical procedures, etc. as required by physicianโ€™s orders, are within the accepted scope of nursing practice.
  • Assist in the development of general systems relating to the daily operation of medical services (medical policies, procedures, protocol, training, etc.)
  • On ongoing basis, monitor and assess health problems of each resident. As needed, approve and implement written Nursing Plan of Care (NPOC) and annual Nursing Assessment and document outcomes.
  • Understanding of State and Federal regulations that are applicable to the program, including delegating nursing duties, when needed, following OAR 851-45-011. Follow Opportunity Foundationโ€™s established Policies and Procedures and changes as mandated by the Organization.
  • Train staff for treatments and tasks under scope of nursing practice to assure they are able to perform and excel in the care of the residents, as needed.
  • Ensure that all required medical record keeping is accurate, complete and clearly written in a timely manner as it pertains to Residential Policies and Procedures.
  • Assist the ISP Team in determining the individualโ€™s needs for assessments and monitoring by other Health Care professionals.
  • Meet with staff as needed to gather information and prepare a plan for care, including attending staff meetings and ISP team meetings.
  • Coordinate scheduling and attendance of medical appointments. Attend relevant appointments. Coordinate contacting physicians, as needed.
  • Attend required and regularly scheduled meetings, including inservice training, team meetings, etc.
  • Participates in Fire Drills and safety checks as needed.
  • Consistently perform job in a safe manner, follow all program policies pertaining to safety including OSHA standards and best practices, and report all safety concerns using Chain of Command.
  • Performs additional duties as assigned or delegated.

Occasional Job Functions

  • Be fiscally responsible.
  • Assist with personnel issues, such as, orientation, supervision, performance issues as requested.
  • File paperwork (MARโ€™s, NPOC, MSP, physician orders, etc.) as needed and maintain accurate, current information in each residents Therap file.
  • Keep needed forms current and available to staff.
  • May be asked to assist with residents at another site.
  • May be required to obtain Oregon Intervention System (OIS) certification.
Requirements:

Education/Work Experience Requirements

  • Current Oregon RN Nursing License.
  • Two years experience required. Preference given for experience working with adults with developmental disabilities.
  • Consistently maintaining confidentiality is a priority in this position.
  • Possesses excellent oral and written communication skills.
  • Ability to get along with others and recognize the need for diversity and flexibility.
  • Must pass pre-employment and random drug screening
  • Must pass the criminal investigations and background checks required by the Oregon Mental Health Division.
  • Must possess a valid Oregon driverโ€™s license (driving record acceptable to our insurance carrier) and proof of personal automobile insurance.
  • Must be able to obtain CPR and First Aid certification.
  • Must complete 12 hours of job-related inservice training annually.
  • May require weekend, holiday and evening hours.

Working Conditions

Predominantly indoors, adequate heat, air conditioning, and lighting. Some noise factors from residents. Loud voices, toys, running in the house, etc. Supplies/some meals provided. Frequent interruptions and changes in the routine are the norm.
