Job description

Overview

The Case Manager Telehealth provides case management services to individuals in a correctional setting, focusing on coordinating care and supporting successful treatment outcomes. This role designs and manages individualized treatment programs, counsels individuals within scope of practice, and helps address social, legal, and institutional adjustment needs. The position works closely with facility staff, probation, medical, behavioral health teams, and community agencies to ensure coordinated services. The Case Manager maintains accountability for services provided, supports continuity of care, and facilitates reentry-related services.

Responsibilities

  • Design, develop, and coordinate individualized treatment programs to meet offender needs.
  • Coordinate services with facility staff, community agencies, probation, medical, and behavioral health partners.
  • Provide counseling and identify individual challenges to recommend appropriate solutions within scope of practice.
  • Support continuity of care by arranging community appointments and reentry-related services.
  • Maintain accurate documentation, comply with security and confidentiality requirements, and support classification and treatment processes.

Qualifications

Education

  • Graduate from an accredited school of nursing

Experience

  • One (1) year of experience required.
  • Mental Health, Substance Use Disorders, Medication Assisted Treatment, or Case
  • Management in a corrections environment preferred.

Licenses/Certifications

  • Have and maintain current licensure as a Registered Nurse within the state of employment
