Job Summary
Odyssey House Louisiana, Inc. is seeking an experienced full-time Case Manager that is responsible for engaging all clients upon admittance into Odyssey House Louisiana, Inc. (OHL). The schedule for this position is Monday-Friday 8am-5pm. The CaseManager is responsible for determining eligibility for chronically unhousedindividuals to participate in the Odyssey House, LA Inc. (OHL) -PermanentSupportive Housing Program (PSH). The CaseManager must provide Community Psychiatric Support Treatment (CPST);Psychosocial Rehabilitation Services (PSR) and Crisis Intervention to programparticipant(s). The Case Managerprovides tenant-based rental and operating assistance. The Case Manager mustnegotiate LA Medicaid system(s). The Case Manager will also be responsible fordocumenting case notes and programmatic activities, which are pertinent to LAMedicaid system(s). The Case Manager isresponsible for monitoring client's progress and needs, discharge plan, andtracking participants who have exited the program. The Case Manager isresponsible for entering data into the agency's assigned data bases andprepares weekly, monthly, quarterly and annual reports reflecting suchactivity, as indicated. The Case Manager is responsible for marketing the (PSH)program, recruiting perspective participants, attend all pertinent trainings.This position is under the supervision of the Project Manager.
Responsibilities and Duties
It is the duty and responsibility of the Case Manager to:
  • Provide casemanagement to approximately twenty-five (25) participants as assigned; Case Managementwill involve screening, intake, assessment, service plan development, monitoring,linkage to appropriate community resources, follow up, appropriate discharge,tracking, etc;
  • Assistparticipant(s) with completing lease agreements and obtaining other supportive documentssuch as identification, social security card, etc as required by leasing agents/property;
  • Assess client(s)Medicaid status and follow agencies policy and procedures to obtain andmaintain Medicaid status;
  • Schedule andperform efficient client...office and home visit(s) that are geared towards(CPST; PSR and Crisis Intervention) Services;
  • Enterparticipant's information into the appropriate agency's database(s) in a timelymatter and update as needed;
  • Prepare andsubmit reports to Project Manager in a timely manner;
  • Maintain up todate information regarding tenant occupancy, move outs, and vacancies;
  • Facilitate andcoordinate supportive activities with other program staff including employmentassistance activities, such as job readiness training, resume preparation, interviewpractice, attending job fairs and networking with perspective employers, etc.
  • Conduct qualityassurance program, self-monitoring plan as assigned by Project Manager;
  • Establish andmaintain collaborative working relationships with homeless shelters and communityresources
  • Attendappropriate coalition and other community resource meetings
  • Attend trainingand workshops as they come available
  • Developpromotional strategies to inform the community about the program includingcommunity presentations, print material, and related activities; and
  • Perform otherduties as assigned.

Qualifications and Skills
Minimum qualifications include the following:
  • Valid Driver's License
  • High school diploma or equivalent
  • 3 years successful experience in adult case management or equivalent experience
  • Ability to work with homeless individuals of diverse backgrounds, and disabilities including physical, mental, substance abuse, HIV/AIDS, etc.
  • Outstanding organizational, verbal, and written communication skills
  • Ability to adapt to change in a fast-paced environment
  • Ability to perform daily duties to achieve expected outcomes and performance
  • measures with minimal supervision
  • Ability to travel to various sites daily

Preferred qualifications include the following:
  • Undergraduate degree in social work or behavioral science
  • Experience in LA Behavioral Health Partnership Billing
  • Experience in providing Community Psychiatric Treatment; Psychosocial Rehabilitation and Crisis Intervention
  • Knowledge of AA/CA/NA/Faith Based Principles
