Schedule: Day shift | Part-time |Weekends

Salary range: $27.69 - $40.50 per hour

How you’ll make an impact in this role
  • Oversee primary needs of patient, including assessment of client and family/caregivers, and develop a comprehensive plan of care.
  • Provide primary home care/consultation to complex client and family needs within area of expertise and in collaboration with the physician.
  • Schedule: 8:00 a.m.- 4:30 p.m, every Friday, every other Saturday, rotating Sundays and weekdays. 
     

What minimum requirements you’ll need

Licensure / Certification / Registration:

  • Counselor credentialed from the Illinois Department of Financial and Professional Regulation preferred.
  • Social Worker credentialed from the Illinois Department of Financial and Professional Regulation preferred.
  • Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Education:

  • High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required.

What additional preferences we're seeking
  • Master's Degree required  
  • Experience in Case Management and Discharge Planning is ideal
  • Weekend availability
