Description

The health services that became the Anthony L Jordan Health Corporation (Jordan Health) began more than 100 years ago, in 1904. As one of the first 5 Federally Qualified Health Centers (FQHC) established in the nation, its roots are steeped in service to those who face barriers to health and health care, meeting their need for comprehensive medical, dentistry, behavioral health, and community services. Starting in northeast Rochester, NY, Jordan Health has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants of Rochester, NY and in Canandaigua, N.Y. 


 Jordan Health is now seeking a Behavioral Health Counselor who, under the guidance of the Director of Social Emotional and Mental Health, is primarily responsible for providing MH assessments, offer family and collateral assistance, crisis intervention, liaison with medical providers, regarding Behavioral Health issues and referrals. Offer short term, evidence-based practice interventions to persons with a range of behavioral health disorders in an integrated primary care/behavioral health system.

Requirements

The Behavioral Health Counselor will ensure to:

Provide comprehensive diagnostic evaluations of mental health disorders based on DSM clinical criteria. Additionally, provide risk assessments as requested by medical providers. 

Assess and develop treatment plans to include medical and behavioral health concerns, which are to be updated, as appropriate, each session with patient.

Help restore social conditions to enhance the patient/family's capacity to meet their personal and social needs.

Makes internal and external referrals to programs and agencies to foster/support resources to patients and families struggling with behavioral health disorders. 

Provides Behavioral Health consultation to medical providers within the organization.

Work in collaboration with medical providers, care managers, and all members of the Behavioral Health staff to provide comprehensive, holistic care to patients and families. 


 Education And Experience Required:

LCSW-R, LCSW-R, LCSW OR LMHC with at least 2 years providing therapy for culturally diverse patients in a clinical setting.

Experience working with culturally diverse patients and Pediatric Behavioral Health desirable.


 Licenses And Certifications: 

LCSW-R, LCSW, and LMHC

BLS through the American Heart Association 

NYS Infection Control Certification 


 Special Skills, Knowledge Required:

Able to diagnose Behavioral Health disorders and make decisions as to an appropriate course of intervention, counseling and referral.

Must have reliable transportation.
