Job description

Job Title: BEHAVIORAL HEALTH CASE MANAGER
Reports To: Behavioral Health Program Supervisor
Scheduled Hours: Full-Time (37.5 hrs. per week)
FLSA Status Exempt
Purpose of Position:
Provide community based mental health case management services to mentally ill individuals who are living in the community (age 55 or over). Provide assessment, treatment planning, linking, monitoring, advocating, consultation, and intervention services for the Milestone Behavioral Health (BH) program. As professional licensure allows, provide as-needed backup coverage services for the OBRA and Nursing Home Monitoring (NHM) programs which provide services to mentally ill individuals who are residing in a nursing home (all ages).

Essential Functions:
  • Carries a caseload for the BH and/or NHM programs. At the direction of the Clinical Manager, offers community based BH services to consumers referred by ISK and/or offers nursing home based NHM services to those identified by an OBRA assessment as needing mental health intervention.
  • Case manages consumer’s overall delivery of service as stated in the Individual Plan of Service to ensure the comprehensive meeting of the consumer’s total unmet needs. This may include transporting the consumer in the case manager’s vehicle.
  • Collaborates with the appropriate medical, psychiatric in-patient, and ISK staff to facilitate the consumer’s discharge and safe return to the community from psychiatric hospital stays.
  • Provides consultation and education to family members of consumers to facilitate their understanding of the primary consumer’s needs and assist them in improving or preserving their involvement and relationship with the consumer.
  • Provides case consultation, staff education, training and support to AFC staff, nursing facility staff, or other involved service providers to increase their awareness of the consumer’s physical and mental health needs related to planned services.
  • Completes all documentation in a timely manner as required by funders and regulatory agencies. Interacts with consumers served, legal representatives, natural supports, AFC staff, nursing facility staff, referral agents, colleagues, funder sources, and other community agents in a positive and professional manner.
  • Participates in all required team and agency meetings, in-services, and trainings. Maintains professional Continuing Education requirements and State of Michigan licensure.
  • Complies with all federal, state, licensing, agency, and funder requirements.
  • Other duties as assigned.
Decision Making Authority:
Most decisions are made by the incumbent in conjunction with established guidelines from the Clinical Manager of Mental Health Services, Integrated Services of Kalamazoo guidelines, and established MDHHS program policies and procedures. Routinely consult with Clinical Manager of Mental Health Services regarding overall program success and areas of concern.
Education/Experience:
  • The position requires a minimum of a Bachelor’s degree in a human service field plus one year of experience working with people with mental illness.
  • Preference will be given to persons with a current state certification or licensure in one of the following Michigan Licensures: RN, LLBSW, LBSW, LLMSW, LMSW, LLPC ,LPC, LLP, LP.
  • Preference will be given to persons with a combination of public mental health system experience and working with the elderly for at least two years.
  • Preference will be given to candidates who have mental health case management experience working within the local CMH network of provider agencies and who are familiar with ISK service standards.
Competencies:
  • Rigorously exercise caution, sensitivity, and good judgment in dealing with consumers, their families, other service providers, and the community at large.
  • Managing close working relations with numerous staff and outside agencies to ensure continuity of care.
  • Ability to efficiently prioritize and process numerous tasks in a moderate to fast paced work setting. This involves the need to be well organized and thorough.
  • Must have reliable personal transportation plus a State of Michigan driver’s license in good standing.
  • Good verbal and written communication skills.
  • The ability to work with older adults and adults with disabilities of a diverse cultural, social, and economic background.
  • Computer and typing proficiency.
  • Perform a variety of procedures to assist consumers including the ability to lift and carry twenty (20) pounds, climb stairs, kneel, stoop, bend, carry packages, baskets, boxes, and bags to assist clients.
