Description
  • Conduct thorough evaluations of patients in both inpatient and outpatient scenarios, crafting tailored treatment plans that meet individual needs

  • Work collaboratively with nurses, physicians, case managers, and other rehabilitation professionals to enhance patient outcomes

  • Accurately document evaluations, therapies, and patient progress as per facility and regulatory requirements
  • Engage actively in care team meetings and interdisciplinary conferences to ensure comprehensive patient care
Education
  • Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution
Certifications
  • State licensure as a Physical Therapist
  • Preferred CPR certification
