Position Summary:

The Care Management Educator assists in the educational needs assessment and development of an educational plan for the Care Management Department. This role will develop and implement robust onboarding plans for new care management team members. Responsibilities: onboarding new hires, providing education to standardize documentation and processes/workflows for Care Management, auditing team member documentation, and Care Management process compliance.

Responsibilities

Essential Functions:

  • Facilitates Care Management training for operational areas.
  • Collaborates with the Corporate Learning Specialist to create a robust onboarding process for new team members.
  • Assists in standardizes new employee orientation and maintains training materials and competencies.
  • Assists in creating clinical competencies for all levels of clinical staff.
  • Assists with Care Management preceptor development.
  • Performs regular audits of Care Management team member documentation for performance improvement activities and educational purposes.
  • Develops educational programs in collaboration with the Corporate Learning Specialist, clinicians, and Care Management Leadership Team
  • Provides general and specialty education to prepare care management staff to be safe, efficient, and competent.
  • Represents department as point person to organize training requests, schedules, and venues.
  • Demonstrates excellence in presentation skills, capturing audience attention and achieving high engagement from the team.
  • Gathers performance feedback for managers on students' achievement during training.
  • Collaborates with Physician Advisors to identify educational opportunities.
  • Collaborates with the Corporate Learning Specialist to ensure new processes are documented for training purposes.
  • Proficient in the use of all Care Management applications
  • Effectively communicates and serves as a subject matter expert on Care Management applications.
  • Maintains reasonably regular, punctual attendance consistent with Orlando Health policies, the ADA, FMLA, and other federal, state, and local standards.
  • Maintains compliance with all Orlando Health policies and procedures.
  • Assists the Corporate Learning Specialist with developing new education/information, maintaining the Care Management SharePoint site and other duties of the learning team.

 

Other Related Functions:

 

  • Serves as a liaison and informal consultant to other site departments regarding educational efforts.
  • Participates in process improvement activities as assigned.
  • Always demonstrates a positive attitude and professional appearance.
  • Acts as a resource for team members
Qualifications

Education/Training:

  • Graduate of an approved school of nursing OR a Master's degree from an accredited school of Social Work, Mental Health, or Psychology is required.
  • RN must have experience in utilization management and proficient with InterQual and MCG.

Licensure/Certification:

  • Florida RN license required and maintained, if graduated from an approved school of nursing.
  • BLS/Healthcare Provider Certification within 90 days of hire.
  • Maintains Florida state (Chapter 491 Florida Statutes) licensure as a Licensed Clinical Social Worker or Licensed Mental Health Counselor if licensed in these fields, or obtains licensure within two (2) years of hire.

Experience:

  • Three (3) years of experience in acute hospital care management, care coordination and utilization management.
  • Effective written and verbal communication skills.
  • Excellent presentation skills.
