The Nurse Navigator (NN) acts as a multidisciplinary team leader and liaison between patients, family, physicians, clinical care teams, community organizations and BHSF leadership. The NN is responsible for leading coordination of care across the continuum, providing education, support and resources to patient/family, facilitate shared decision making, patient advocacy, ensuring timely access to quality multidisciplinary care and patient satisfaction. The NN will monitor individuals, populations and program outcomes and engage in quality improvement activities.
Qualifications:
