Job description

Job Objective:

The RN Navigator - Emergency Department (ED) is a Registered Nurse, responsible for overseeing the ED Waiting Room to enhance ED throughput, patient escalation as needed, while improving patient experience. The RN Navigator is the "face of our ED" and is the person that will be able to impact the movement of our patients through strong collaboration with the entire health care team.

Duties and Responsibilities:

  1. Nursing Process
  1. Applies nursing process in direct patient care:
    1. Provides accurate assessment and documentation of physiological, psychosocial, cultural, developmental, socioeconomic, and spiritual needs of patient as evidenced by assessment within the time frame appropriate for level of care.
    2. Provided information to patient/family relating to emergency room care/treatment and keeps them informed on next steps along the continuum of care in the ED.
  • Adheres to all current policies and procedures in providing patient care and seek clarification if needed.
  1. Coordinates with the multidisciplinary team to promote awareness of barriers to care and to identify mechanisms to overcome those barriers.
  2. Remains a point of contact for the patient along the continuum of care in the ED.
  3. Incorporates time management skills in accomplishing care and giving direction to follow up on delays in care or treatment.
  1. Greets patients/families entering the ED Waiting room and helps to direct them to the Registration Windows or escalate for immediate care as driven by their presentation.
  2. Remains engaged with the patient/family in the ED Waiting Room and is the source to help them navigate timely through the steps of care/treatment in the Waiting Room
  3. Ensures Vitals Signs are completed every 2 hours while sitting in the Waiting Room
  4. Engage with the patient/family to ensure their care and reported treatment results are moved timely to the providers to make a decision on final disposition.
  5. Ensure discharges are done timely; assist with having the provider and nursing staff complete the discharge paperwork and discharge the patient timely.
  6. Prevent Left Without Being Seen to communicate with the Triage RN and Provider in the ED Waiting Room long delays.
  7. As needed assists as a 2nd Triage RN to move the patients along sooner to the Provider
  8. Documents patient care and follow-up as required in the Electronic Medical Record.
  9. Assists with locating the patient when they are called for triage/treatment/follow-up in the ED so the patient is not overlooked.
  1. Nursing Care Delivery Process:
  1. Knowledgeable regarding current nursing practices and procedures
    1. Demonstrates knowledge and use of resources and forms
    2. Demonstrates knowledge of hospital structure and administrative resources
  • Demonstrates ability to interpret rules and regulations
  1. Knowledge of legal aspects of nursing care and EMTALA laws for the ED.
  2. Demonstrates knowledge of Patient Rights
  3. Acts as a liaison between the patient and the healthcare team.

  1. Maintains professional attitude and effective working relationships with the healthcare team.
  2. Functions appropriately and effectively under pressure. Provides care as part of team effort in crisis or emergency situation.

  • Strong ED operations knowledge and critical thinking ability.
  • Knowledge of common Codes required for quick action in the ED (Code Stroke, STEMI, Trauma Tier qualification, Code Delivery, Code Tan, etc.)
  • Understands EMTALA regulations along with terminology LWBS (Left without Being Seen), LWOT (Left without Triage), Elopement and AMA and appropriate documentation for a discharge disposition.
  • Conforms to required accreditation standards
  • Conforms with and supports hospital quality management and performance improvement guidelines

Education and Experience:

Education

  • Registered Nurse (RN); graduate of an accredited RN school of nursing
  • BSN preferred
  • Successfully completes written and practical competencies annually or otherwise as assigned
  • Demonstrates knowledge and competency on hospital information technologies used in the designated patient care area.

Experience

  • Minimum of Two (2) years Emergency Room RN experience in an acute care setting
  • Triage Certification required
  • MICN certification preferred

Required Licensure and/or Certifications:

  • Current California Registered Nursing License
  • BLS, ACLS, PALS
  • MICN preferred
  • Maintains current licensure, certifications, education, and physical health standards
