  • Weekend - $4.00/hour
  • Lead - $3.25/hour
  • Preceptor - $1.00/hour
  • Per Diem - 15% (on base plus steps, in lieu of benefits)
  • Certification - $2.00/hour
  • Weekend Coord Pay (chg) - 15% on regular pay

JOB DESCRIPTION:
Assesses, plans, implements, coordinates and evaluates the nursing care of patients in their place of residence. Promotes continuity of care for patients on their assigned caseload. Provides direct patient care and supervises the care provided by LPNs and HHAs. Works in collaboration with all home health disciplines assigned to provide care for patients on caseload. Demonstrates a working knowledge of the principles of growth and development of patients 12 years of age and older. Applies principles of learning in teaching patient's self-management and methods to regain independence.
Individuals assigned to this classification are expected to apply full working knowledge of acceptable standards of nursing principles and practices, demonstrate problem-solving skills and make independent decisions in complex and diversified work situations under general supervision. RN Case Managers utilize established departmental policies and procedures and home health regulatory requirements in providing care to patients. Care is provided as prescribed by the patient's physician.
Performance of tasks under emergency conditions may require strenuous exertion (i.e. patient resuscitation in the home). Has contact with patients under a variety of circumstances and is subject to varying and unpredictable situations. Occasionally subjected to irregular hours. Must maintain a keen awareness of personal safety. Provides on-call support as assigned for patients.
EDUCATION
  • Associate's degree required.
  • B.S.N. preferred.

EXPERIENCE
  • Minimum of one year recent experience preferred in home health, acute or long-term care.

LICENSURE/CREDENTIALS
  • Current Washington State Registered Nurse license, required.
  • Current BLS certification, required.
  • Valid Washington driver's license, required.
  • Proof of insurance on automobile used for work, required.
  • Driving record in compliance with OMC standards currently in effect or as may be amended, required. See HR Policy Driving for OMC Business.
  • OMHH issued work cell phone.
  • Must have computer internet connectivity (OMHH work laptop provided).
  • Must have reliable automobile transportation for entire scheduled shift to travel to patient residences.
