Position Overview

The Home Health Registered Nurse provides skilled nursing services to patients in their homes in accordance with physician orders and the patient’s individualized Plan of Care.

The RN is responsible for performing patient assessments, coordinating care, educating patients and families, and ensuring compliance with Medicare, CMS, CHAP, and state regulatory requirements.

This role requires strong clinical judgment, excellent communication skills, and the ability to work independently in the field.


Key ResponsibilitiesPatient Care

• Perform comprehensive patient assessments including OASIS assessments when required
• Develop, implement, and update the Plan of Care in collaboration with physicians and the interdisciplinary team
• Provide skilled nursing treatments and interventions as ordered by the physician
• Monitor patient progress and document outcomes of care

Coordination of Care

• Communicate patient status and changes to physicians and the care team
• Coordinate services with therapists, aides, and other healthcare professionals
• Ensure timely follow-up on physician orders, lab results, and care updates

Documentation & Compliance

• Maintain accurate and timely clinical documentation
• Ensure compliance with CMS, Medicare, CHAP, and agency standards
• Participate in clinical reviews, quality assurance, and chart audits when requested

Patient & Family Education

• Educate patients and caregivers on disease management, medications, and safety
• Provide guidance on preventative care and recovery strategies

Team Collaboration

• Supervise LPN/LVN staff and home health aides when appropriate
• Participate in meetings, training programs, and agency in-services
• Support quality improvement initiatives and best practices in home health care

Qualifications

Required

• Active Registered Nurse (RN) license in the state of employment
• Graduate of an accredited School of Nursing
• Valid driver’s license and reliable transportation
• Current CPR certification
