Primary Care Physician (MD/DO) - Job Description 

Employment Type:Full Time, Exempt(30-40 hours per week) 
Location:Highland Park Clinic | 2004 Ford Pkwy, Saint Paul, MN 55116
Compensation: Competitive base salary, quarterly quality bonus eligibility, PTO, CME stipend

We are building a new model of primary care for women 65+ to solve long-standing problems: rushed appointments, long wait times, and care that's generalized rather than specialized towards women's needs later in life such as post-menopausal care, bone density, weight management, and mental, social and emotional well-being.  

Our patient-centric Primary Care clinics are dispersed across Minneapolis/St. Paul, MN. We are thrilled to serve the Twin Cities metro in our clinics located in Highland Park (St. Paul), Crystal, and South Minneapolis, Rosedale and our newest clinic in Eagan. Our team of 100+ colleagues is on a mission to innovate the primary care landscape for women 65+. We're seeking like-minded individuals who share in this passion to join us! 

About You:

Our ideal candidates have strategic prowess and the ability to use data to build best practices and implement great ideas in collaboration with our team and our community. 
The Primary Care Physician's Role: 
The Primary Care Physician (MD/DO) is a key member of our interdisciplinary care team. In the role, you will evaluate our patients' acute and chronic conditions, manage their multiple medications, mental health, socio-emotional needs, and provide empathetic patient education. The PCP will work closely with fellow clinical teammates to provide comprehensive primary care. While direct patient care is the primary focus of the position, our PCPs also assist with development, implementation and achievement of key organizational objectives and metrics. Functioning as a value-based care model, our team of clinicians proactively and strategically manage our population's health in accordance with IHI's Quintuple Aim, adopt best practices in coding compliance and achieve our HEDIS quality metrics. 

Core Qualifications: 

  • Licensed MD/DO with board certification with MBPS recognition (IE ABFM, ABIM, etc)
  • Minimum 3-5 years of experience managing complex disease longitudinally over time including diabetes, cardiovascular disease, menopause, thyroid disease, osteoporosis, weight management, etc.
  • Understanding or willingness to learn how to practice in a VBC model-including managing the cost and quality of care 
  • Experience managing overall patient care and devising individualized care plans
  • Experience educating and counseling patients on health maintenance and disease prevention
  • Experience with geriatric medicine, particularly women
  • Experience with poly-pharmacy management and appropriate medication reduction
  • Experience with physical and gynecological/well-woman examinations in the elderly population
  • Experience working in clinical situations that required adaptability
  • Strong problem-solving and critical thinking skills
  • Up-to-date knowledge of medical advancements and technologies
  • Alignment to Herself health mission and dedication to providing high-quality patient care  

Preferred Qualifications: 

  • Experience in performing clinical procedures within scope of practice
  • Value-based Care experience and/or VBC training
  • Previous experience in a start-up or new clinic environment
  • Strong technical skills with EMR (Athena preferred) and Microsoft Suite
  • Experience in mentoring, precepting and/or supervision of other provider staff 

Other Qualifications: 

  • Eligibility and willingness to obtain a Minnesota MD or DO license and DEA
  • Eligibility to obtain or holder of a DEA License and an NPI number
  • Regular maintenance of board certification and CME requirements
  • Passage of the pre-employment background check
  • Eligibility and completion of registration/credentialing with Medicare and private payers
  • Valid Minnesota Driver's License
  • Proof of US Citizenship or Eligibility to Work in the US with a permanent resident card
  • Employees in direct patient care are expected to be fully vaccinated for all communicable diseases including Hepatitis B, MMR, influenza and COVID in accordance with CDC guidelines.
