We are seeking a compassionate and skilled Registered Nurse (RN) to join our team and serve patients from Imperial Beach to Santee. This role involves planning, organizing, and coordinating home care services, with a strong emphasis on community health and patient education.

Key Responsibilities

  • Develop and initiate individualized care plans in collaboration with patients and caregivers
  • Deliver physician-ordered nursing services and implement preventive and rehabilitative procedures
  • Educate patients and caregivers to promote health and independence
  • Identify and implement discharge planning needs
  • Coordinate necessary equipment and services
  • Supervise LPNs, LVNs, and HHAs

Qualifications

  • Active California RN license
  • Minimum 1 year of direct patient care experience (home health preferred)
  • Current CPR certification
  • Valid driver's license and insured, reliable vehicle
