Monday - Friday 8:30am-5:00pm w/alternating weekends
*Medicare Home Health experience preferred*

Job Summary:
The RN utilizes the nursing process of assessment, planning, intervention, implementation and evaluation. The RN is skilled at assessment and management of pain and other complex symptoms, including education and support to patient and family. The mission of the home health team is to meet the unique needs of clients and families with services tailored to assist them in achieving optimal level of function.
Job Duties:
  • Initial and ongoing comprehensive assessment of the client's needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate time points.
  • Initiating the plan of care and revising as necessary. The home health care plan is designed to the unique needs of clients and families with services tailored to assist them in achieving optimal level of function.
  • Providing those services and/or treatments requiring substantial and specialized nursing skill.
  • Counseling the client and family in meeting nursing and related needs.
  • Initiating appropriate preventative and rehabilitative nursing procedures.
  • Preparing clinical and progress notes.
  • Coordination of services.
  • Referral to other services as needed.
  • Informing the physician and other staff of changes in the client's needs.
  • Evaluating outcomes of care.
  • Supervision of LPN/LVNs and paraprofessionals providing services. Assigning home health aide to specific patients.
  • Participating in in-service programs.
  • Planning for the discharge from service.
  • Responsible for implementing HHA care plan and supervisory visits.
  • Responsible for the observation and assessment, skilled procedures, teaching to families/caregivers.
  • Participate in bi-weekly case conferences.
  • Participates in any QA programs as needed.
  • Will follow HIPAA guidelines at all times safeguarding the privacy of patients protected health information.
  • All other duties as assigned.

Education/Experience:
Graduate of an accredited nursing school. One year medical/surgical RN experience.
Certificates, Licenses, Registrations:
Valid Florida Driver's License.
Current Registered Nurse License to practice in the state of Florida
Healthcare Provider CPR required upon hire and maintained.
Supervisory Responsibilities: May supervise LPNs and Home Health Aides as appropriate.
Computer Skills:
Knowledge of basic typing skills, data entry and word processing software, database software and general knowledge of e-mail.
Language Skills:
Ability to read, analyze, and interpret common scientific and technical journals. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community.
Mathematical Skills:
Ability to work with mathematical concepts such as probability and statistical inference. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
Reasoning Ability:
Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.
