Transitions At Home is a trusted provider of home health services, committed to delivering high-quality, compassionate care to patients in the comfort of their homes we are seeking an experienced RN Case Manager to support our care team and play a vital role in coordinating and delivering patient-centered care..
If you are a caring, compassionate, critical thinking RN who has experience in Home Health, ICU, ER and Skilled Nursing Homes who would appreciate and enjoy a flexible work schedule, independence, and appreciates a work family team culture, we would love to add you to our Transitions At Home Team.
What does a Transitions at Home RN Case Manager do? Transition at Home RN will provide expert guidance and oversight in clinical and regulatory matters. This role includes staff training, critical thinking, care plan review, incident investigation, and Who also coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patientโ€™s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.
Hours: Full - Time
  • 40 hours per week 8:00 am - 4:30pm
  • Monday through Friday
  • Weekends or Holidays
  • Limited on-call rotation

Qualifications / Job Requirements:
  • Active Wisconsin Registered Nurse License
  • Two (2) yearsโ€™ nursing experience, 1 year in Home Health (preferred)
  • CPR Certification
  • Wound Care would be beneficial
  • A valid Wisconsin Driver's License and Car Insurance
