What is an at home RN?

At Home RNs, also known as home health registered nurses, are licensed nurses who provide medical care to patients in their own homes. They typically assist patients recovering from illness, surgery, or injury, and may also care for those with chronic health conditions. Their responsibilities include administering medications, monitoring vital signs, educating patients and their families, and coordinating with other healthcare professionals. At Home RNs help ensure patients receive quality care outside of a traditional hospital or clinic setting, promoting recovery and independence.

What are some common challenges faced by an at home RN, and how can they effectively manage them?

At Home Registered Nurses (RNs) often encounter challenges such as working independently without immediate access to colleagues, managing diverse patient needs in non-clinical environments, and adapting to varying home conditions. To effectively manage these challenges, At Home RNs should maintain strong communication with their healthcare team, utilize telehealth tools for support, and stay organized with thorough documentation. Building rapport with patients and their families is also crucial to ensure smooth care delivery and address any concerns promptly.

What are the key skills and qualifications needed to thrive as an at home RN, and why are they important?

To thrive as an At Home RN, you need strong clinical assessment skills, nursing credentials (such as an active RN license), and experience in home health care. Familiarity with telehealth platforms, remote patient monitoring systems, and electronic health records is typically required. Exceptional communication, independence, and time management are vital soft skills for working autonomously and supporting patients in their own environments. These competencies ensure safe, effective care delivery and foster trust while adapting to the unique challenges of home-based nursing.

They often handle chronic disease management, medication monitoring, and health coaching, requiring relevant licensure and strong communication skills.

Job description

Coverage Territory: Effingham and surrounding areas

Schedule: 8 AM–8 PM, alternating 4-day and 3-day weeks including weekends in rotation

Make the Moments That Matter Most

At Transitions Hospice, we don't just manage care — we protect dignity, comfort, and the moments that matter most. We've reimagined hospice to be something different: compassionate, unhurried, and built around the whole person. That means 24/7 care delivered where patients feel safest — at home — with physical, emotional, and spiritual support that lets them focus on living.

If you're an RN who wants your work to mean something beyond a chart, this is your role.

What You'll Do

As an RN Case Manager, you'll be the clinical anchor for your patients and their families — coordinating care, guiding the interdisciplinary team, and showing up when it matters most. Your day-to-day includes:

  • Conducting comprehensive, ongoing assessments to understand each patient's evolving needs
  • Building and adjusting personalized care plans in collaboration with physicians and care teams
  • Administering medications and treatments per physician orders
  • Keeping clinical documentation accurate, timely, and meaningful
  • Serving as the communication bridge between families, physicians, and community health partners
  • Educating patients and caregivers so they feel confident and supported
  • Participating in on-call rotation per team policy

Benefits We Offer

  • Competitive pay
  • Medical / Dental / Vision Insurance plans
  • Long & Short-Term Disability
  • 401(k) with company match
  • Auto Allowance & Gas Card
  • Company provided equipment
  • Paid Time Off

Why Choose Transitions Hospice?

Hospice nurses at Transitions don't just manage cases — they change lives. We give you the tools, support, and team to do your best work, without sacrificing the human side of care. Our nurses are the difference, and we treat them that way.

Ready to join us? Apply today.

What You Bring

  • Active RN license in the state of employment
  • Graduate of an accredited school of Nursing.
  • One (1) + year Hospice, home health or related organization experience preferred.
  • Compassion for patients and families navigating end-of-life care
  • Strong clinical judgment and the ability to work independently in the field
  • Excellent communication skills across care teams and families
