Home Health Services serving various communities.
Typical caseload involves an average of 6-7 visits per day and approximately 30 visits per week.
Admissions are weighted at 2.40 units, and 2 admissions can be expected per day.
Candidates must have experience with patients aged 18 and older.
Common patient diagnoses include wound care, wound vacs, Pleurx drains, Foley catheterizations, feeding tubes, and IV infusions.
Experience with Diabetes Mellitus and Heart Failure is necessary.
Candidates should be skilled in medication and disease management.
Required skills include Homecare Homebase charting experience, Oasis, hospice philosophy, infection control practices, and wound care management.
Experience with PICC line dressing changes, port access, IV infusions, and catheter insertion is essential.
Candidates must be able to drive at night.
Department support includes leadership, LPN/CNAs, and case management.
Technological tools used include EMR: Homecare Homebase.
Scheduling is Monday to Friday from 8 AM to 5 PM.
Weekends and holidays are dependent on needs and will be discussed during the interview.
Dress code mandates navy scrubs.
