Job Overview
Priority Home Care caregivers play an important role in supporting individuals during some of the most important transitions in their lives — aging at home, recovering after medical events, and adjusting after the arrival of a new baby. Our caregivers help bridge the gap between medical care and home stabilization, ensuring clients remain safe, supported, and independent after facility care ends.
This role goes beyond basic caregiving. Caregivers become part of the support system that helps prevent setbacks, reduce caregiver burnout, and provide stability during recovery and life transitions.
Care may include supporting:
Senior Care
Helping older adults remain independent through companionship, safety supervision, daily routine support, mobility assistance, and help with everyday activities that allow them to remain safely at home.
Postpartum Support
Assisting mothers recovering after delivery by helping maintain a stable home environment, supporting recovery needs, assisting with light household tasks, and providing an extra layer of support during the early weeks of adjustment.
Post-Hospital / Rehab Discharge Support
Helping individuals transition safely home after hospitalization, rehabilitation, or skilled nursing stays by reinforcing care plans, assisting with daily activities, helping maintain routines, and observing changes that families or providers may need to be aware of.
Caregivers help provide:
• Stability during recovery periods
• Support for daily structure and routines
• Fall prevention awareness
• Observation and communication of concerns
• Companion support to reduce isolation
• Relief support for family caregivers

Experience
- Previous experience in healthcare is preferred but not required.
