Associate Clinical Social Worker (ASW)

Location: Sacramento, CA

Job Type: Full-time

Shifts: Mon - Fri (Days)

Setting: FQHC or community health outpatient clinic


Job Summary:

The Associate Clinical Social Worker (ASW) provides integrated behavioral health services within a primary care setting using the Primary Care Behavioral Health (PCBH) model. This role focuses on delivering brief, evidence-based interventions, supporting same-day patient needs, and collaborating closely with medical and behavioral health teams to ensure coordinated, patient-centered care.

Roles & Responsibilities:

  • Provide same-day "warm handoffs" from medical and referral teams.

  • Deliver brief, evidence-based behavioral health interventions and psychoeducation.

  • Conduct risk assessments, safety planning, and crisis intervention within scope of practice.

  • Collaborate with primary care providers (PCPs), medical assistants, and behavioral health team members.

  • Participate in case consultations and communicate treatment recommendations to care teams.

  • Facilitate referrals to internal and community-based services.

  • Perform behavioral health screenings and assess patient needs.

  • Provide short-term, solution-focused therapy under supervision in alignment with PCBH model.

  • Maintain accurate and timely documentation of patient encounters, consultations, and communications.

  • Manage behavioral health inbox, voicemail, and appointment scheduling.

  • Conduct outreach for follow-ups, welfare checks, and care coordination.

  • Support patients in accessing higher levels of care when needed.

  • Participate in team meetings and quality improvement initiatives.

  • Ensure compliance with HIPAA, privacy regulations, and ethical standards.

  • Promote culturally responsive care, patient engagement, and retention.

Qualifications & Experience:

  • Master of Social Work (MSW) from an accredited program.

  • Active Associate Clinical Social Worker (ASW) registration in good standing with the California Board of Behavioral Sciences.

  • Experience providing brief, evidence-based interventions in clinical or community settings (including internships).

  • Experience in behavioral health integration or medical settings preferred.

  • Familiarity with the Primary Care Behavioral Health (PCBH) model is a plus.

  • Experience working with diverse and underserved populations preferred.

  • Minimum 1 - 2 years of experience is required

  • Bilingual or multilingual proficiency in English and one of the following: (preferred)

  3. Farsi

  4. Pashto/Pushto
