JOB DESCRIPTION:

The Visit RN helps provide continuity of care by providing nursing visits for care of patients as delegated by the Case Manager RN who remains responsible for the overall planning and coordination of care for a caseload of patients. The Visit RN member of the interdisciplinary team provides nursing care to the patient in the patient's place of residence as prescribed by the physician and in compliance with applicable laws, regulations and policies of the agency. Keeps the Case Manager RN informed about patient progress, recommended actions and physician orders, and plans for the next visits.

Individuals assigned to this classification are expected to apply full working knowledge of standards of nursing principles, practices, and to demonstrate problem-solving skills and make independent decisions in complex and diversified work situations under general supervision.

Performance of tasks under emergency conditions may require strenuous exertion (i.e. patient resuscitation in the home). Has contact with patients under a variety of circumstances and is subject to varying and unpredictable situations. Occasionally subjected to irregular hours. Must maintain a keen awareness of personal safety. Visit RN's provides on-call support as assigned for patients. Per diem Visit RN's participate in on-call rotation on a voluntary basis.

EDUCATION

  • Associate's degree required.
  • BSN preferred.

EXPERIENCE

  • Minimum of one-year recent experience preferred in home health, acute or long-term care.

LICENSURE/CREDENTIALS

  • Current Washington State Registered Nurse license.
  • Current BLS certification required.
  • Valid Washington State driver's license required.
  • Proof of insurance on automobile used for work required.
  • Driving record in compliance with OMC standards currently in effect or as may be amended, required. See HR Policy Driving for OMC Business.

OTHER

  • OMHH issued work cell phone provided.
  • Must have computer internet connectivity (OMHH work laptop provided).
  • Must have reliable automobile transportation to travel to patient residences.
