Summary:

The SBHC Registered Nurse has the responsibility and accountability for assessing patient/family health problems and resources, taking a leadership role in the planning, implementation and evaluation of the nursing plan of care provided by the nursing team. Providing technical nursing care and interventions to a designated patient populations. Per the Departments Scope of Practice, this position requires providing services to Pediatric and Adolescent Patients, ages four to 20 years of age in a manner that demonstrates an understanding of the functional/developmental age of the individuals served. This position requires that the individual be able to work independently without direct supervision on site. This position requires the full understanding and active participation in fulfilling the Mission of CHRISTUS Health. It is expected that the associate demonstrate behavior consistent with the Core Values. The associate shall support the CHRISTUS strategic plan and the goals and direction of their Performance Improvement Plan (PIP)

Responsibilities:

  • Demonstrates the ability to accurately access and document patient care activies and complies with hospital policy.
  • Establishes presence of parental consent prior to assessment, treatment or procedure.
  • Documents in the medical record according to policy/procedure.
  • Chart audits reflect appropriate documentation.
  • Performs assessments and reassessments/care according to RN guidelines and at intervals as required or appropriate to the patient's needs.
  • Correctly differentiates between normal and abnormal findings.
  • Ensures spiritual assessment/care includes actual/expressed needs.
  • Identifies and documents patient/family educational needs upon initial assessment and as needed.
  • Makes appropriate referrals for positive high-risk screens.
  • Follows up on referrals.
  • Complies with incident reporting and notification requirements. Hospital and SBHC.
  • Practices Standard Precautions in all patient care activities.
  • Adheres to medication policies, practices and standards.
  • Appropriately handles and disposes of sharps.
  • Maintains OTC medications to meet pharmacy requirements including completion and faxing of monthly OTC pharmacy inspection document.
  • Administers over counter medications as designated by parental consent form and medical director guidelines.
  • Follows school board policy regarding daily medication administration.
  • Assures the rights of patient and family including, privacy and modesty in the provision of care.
  • Reports suspected cases of abuse/neglect, if identified.
  • Develops, implements and evaluates a nursing plan of care for the student.
  • Develops and implements the plan of care based on assessment findings and sound clinical judgment. Re-evaluates and modifies the plan of care, based on the patient's response to the interventions.
  • Prioritizes patient care activities based on patient assessment.
  • Appropriately coordinates and/or delegates aspects of the plan of care.
  • Involves the patient/family in developing the plan of care.
  • Identifies barriers to learning and provides teaching based on identified needs.
  • Collaborates with school nurse to evaluate immunization record of the student enrolled in the Health Center and administer immunizations as needed / age appropriate.
  • Acts as a medical liaison between the school, parents, and the community regarding health and safety of identified population.
  • Provides classroom educational presentations or facilitates speakers for students on preventive health issues.
  • Demonstrates knowledge of unit specific policies and procedures and the ability to safely and competently use unit specific patient care equipment in the delivery of quality patient care.
  • Completed Unit Specific Annual Competency Checklist.
  • Obtains necessary training prior to initial equipment use.
  • Assures equipment is in operating order prior to use.
  • Differentiates between patient complications and equipment malfunction.
  • Uses medical equipment in accordance with manufacturer's operating guidelines.
  • Adheres to the policies, practices and standards regarding lab testing.
  • Attends hospital-wide in-services to maintain current knowledge on related issues.
  • Initiates contact with Director of SBHC for training on appropriate topics related to health center.

Requirements:

  • Excellent written and verbal communication skills English required.
  • Two-Four years Pediatric experience preferred.
  • Current Louisiana RN License
  • Current BLS Card
