Job Description Summary:
Responsible for facilitating all referrals/transfers and bed coordination for the West Tennessee Healthcare system. Responsibilities include triaging transfers and admissions as indicated by patient condition/diagnosis and physician request. Employee will act as a liaison and resource person for personnel for their area of expertise. Must have excellent verbal communication skills and act in accordance with the WTH Code of Conduct. Incumbent is under the general direction of the Director of the West Tennessee Healthcare Regional Transfer Center, responsible for an assigned shift. Current RN licensure as approved for practice in the State of Tennessee. Five years of continuous/current clinical experience preferred. Employee is subject to call back and overtime as required by the hospital.
ESSENTIAL JOB FUNCTIONS:

  • Facilitates the coordination of all physicians involved in a patient transfer, utilizing phone technology for recording. Completes and maintains all information in Epic.
  • Performs and helps coordinate processes and activities related to bed assignment for indicated areas; interacts appropriately with nursing, physicians/their staff, other departments and Quality Council.
  • Cross-trained for transfer and bed placement duties.
  • Abides by hospital and departmental policy/procedures; reviewing those especially related to transfers/bed assignment and makes recommendations to Director/Executive Director.
  • Receives clinical report for patients coming to the Emergency Department. Records all reports in Epic.
  • Completes the Non Emergent Physician Consult process with physicians and enters consult orders.
  • Makes recommendations related to process improvement for own department and others that may be affected.
  • Demonstrates knowledge and judgement needed to respond to requests for assistance with a wide range of transfer/placement concerns.
  • Demonstrates reliability and dependability by professional behavior and appearance.
  • Maintains licensure, certifications necessary for continued professional growth.
  • Participates in development and maintenance of department budget. Contributes to a positive work climate and the overall team effort of the department and system.
  • Performs related responsibilities as required or directed.

EDUCATION:
  • Skills and proiciency in the principles, concepts and techniques of the nursing profession. Such proficiency as normally acquired through completion of an Associate, Diploma or Baccalauteate degree in Nursing.
LICENSURE, REGISTRATION, CERTIFICATION:
  • Current Tennessee License for Registered Nurse
EXPERIENCE:
  • Minimum of five years clinical experience preferred
  • Exceptions will be viewed on a case by case basis
KNOWLEDGE, SKILLS AND ABILITIES:
  • Must be able to type 45 words a minute
  • Experience using a personal computer with the ability to use computer applications and work within given time frames
