About the Role
Practice nursing with the autonomy, time, and patient connection that allows you to make a meaningful impact every day. As a Home Health Registered Nurse, you'll deliver comprehensive skilled nursing care to patients in their homes, supporting recovery, chronic disease management, and overall health goals while helping individuals remain safely in the comfort of their own environment.
This role is ideal for an RN who values clinical independence, critical thinking, and building strong relationships with patients and families over time. You'll manage a caseload of patients from admission through discharge, providing skilled nursing care, conducting assessments, and developing individualized care plans tailored to each patient's needs. Working closely with physicians and an interdisciplinary care team, you'll coordinate services, make informed clinical decisions, and play a central role in shaping patient outcomes and supporting each person's journey toward greater health and independence.

  • Office Location:
  • Main Service Area: Grand Haven, MI and surrounding areas
    • This position requires daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes
Hours
  • Monday to Friday, 8 a.m. to 5 p.m.
  • Weekend On-Call Rotation
Required Qualification
  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing
  • Current State License as a Registered Nurse
  • Minimum 1 year of experience as a licensed RN in a clinical care setting
  • Reliable transportation, valid driver's license, and valid automobile insurance coverage
  • Willingness to travel daily for home visits within service area
Preferred Qualifications
  • Bachelor of Science in Nursing (BSN)
  • Experience in home health environment
  • OASIS certification or willingness to obtain
