• Maintain precise and timely documentation of clinical interactions through comprehensive clinical and progress notes, adhering to organizational policies.
• Collaborate with interdisciplinary team members to design treatment plans and discharge strategies based on thorough psychosocial assessments and evidence-based interventions.
• Consult and communicate effectively with clients' families and support networks to align on treatment goals and discharge planning.
• Develop individualized treatment plans that incorporate patient history, current circumstances, and treatment objectives.
• Conduct regular sessions with clients to provide counseling, monitor treatment progress, and make necessary adjustments to treatment strategies.
• Administer ongoing evaluations to measure patient progress and adapt interventions as needed to improve outcomes.

This role involves working with patients who are facing complex psychological and emotional challenges, particularly in the context of hospice care. You will interact with clients from diverse backgrounds who may be coping with terminal illnesses, chronic health issues, and related mental health concerns. Your contributions will be crucial in promoting quality of life and supporting families through challenging transitions.

• A minimum of 3 years of relevant professional experience in social work or mental health settings.
• A Master's degree in Social Work, Counseling, or a closely related field is required.
• A valid state license as a Licensed Clinical Social Worker (LCSW) is essential.
• Current Associate or Licensed status with the Board of Behavioral Sciences is required or in progress.
• Demonstrated strong organizational skills and the ability to manage multiple tasks effectively.
• Self-motivated with a proactive approach to initiating billing hours and completing documentation accurately.

• Experience working in hospice, palliative care, or with terminally ill patients is highly preferred.
• Familiarity with various therapeutic modalities and interventions tailored to diverse patient needs.
