Key Responsibilities
  • Directly provide psychotherapy and counseling services to a diverse caseload, focusing on individuals coping with major mental illnesses and emotional disorders, both in client homes and community settings.
  • Facilitate collaboration among multidisciplinary team members to establish integrated care plans tailored to each client's needs, ensuring coordinated access to necessary services.
  • Implement evidence-based screening techniques to assess client needs, developing treatment strategies that prioritize choice and empowerment.
  • Engage in crisis intervention efforts as needed, utilizing best practices to de-escalate situations and provide immediate support for clients in distress.
  • Conduct thorough assessments and follow-ups to monitor client progress, ensuring timely adjustments to treatment plans based on ongoing evaluations.
  • Maintain accurate and timely documentation of client interactions and outcomes, adhering to professional ethical standards and organizational compliance guidelines.
  • Participate actively in team meetings and committees aimed at improving service delivery and enhancing the quality of care provided at Stone Path Counseling.
  • Carry out on-call duties when assigned, ensuring that clients have continued support during evenings and weekends.
Patient Population / Client Focus

In this role, you will work predominantly with underserved populations in the Aurora area, focusing on diverse patient backgrounds with varying cultural, social, and economic situations. Your work will address the unique challenges these communities face, including access to mental health services and socioeconomic barriers. You will play a key role in creating a therapeutic environment that is empathetic and responsive to the clients' lived experiences.

Qualifications
  • Must hold a Bachelor’s Degree in Social Work, with a valid LCSW licensure in the State of Illinois.
  • Expert knowledge of clinical assessments, treatment modalities, and crisis management strategies for a range of mental health disorders.
  • Exceptional verbal and written communication skills, with the ability to connect and build trust with clients, families, and teammates.
  • Strong organizational skills to manage a caseload effectively while adhering to deadlines for documentation and reporting.
  • Competence in addressing age-specific and culturally relevant issues within the client population.
Preferred Experience
  • Minimum of 2 years of clinical experience working in a mental health setting is strongly preferred, emphasizing direct client care and community service.
  • Experience with integrated care models and proficiency in using electronic health record systems is advantageous.
