Job description

Job Title: Licensed Practical Nurse (LPN) Tentative Start Date: December 1, 2025 Work Schedule: Monday–Friday, 8-hour shifts between 0600–1700 , excluding federal holidays Qualifications
  • Education:Graduate of a practical nursing program accredited by a U.S. agency.
  • Experience:Minimum of 3 years of clinical nursing experience within the last 5 years. Experience in referral management, TRICARE systems, or military healthcare environments highly preferred.
  • License/Certification:Current, full, active, and unrestricted LPN license in a U.S. jurisdiction; BLS certification required.
  • Other Requirements:
    • Must be a U.S. Citizen.
    • Proficiency with Microsoft Office (Word, Excel, Access) and EHR systems (GENESIS preferred).
    • Strong communication, organizational, and customer service skills.
    • Knowledge of patient privacy, HIPAA regulations, and medical ethics.

  • Current, full, active, unrestricted license as a Licensed Practical Nursein any U.S. state or territory.
  • Basic Life Support (BLS) certification.
Responsibilities
  • Provide direct patient care under the supervision of physicians and Registered Nurses (RNs).
  • Perform patient assessments, document vital signs, administer medications, and update patient records.
  • Support the Referral Management Team by processing, tracking, and coordinating specialty care referrals.
  • Educate patients and families on treatment plans, medication use, and health benefits.
  • Assist in analyzing referral and utilization data, preparing reports, and ensuring compliance with TRICARE and DHA policies.
  • Act as a liaison between patients, providers, and referral management centers to ensure continuity of care.
  • Maintain accurate nursing and referral records, ensuring timely data entry into EHR systems.
  • Demonstrate knowledge of utilization management and review (UM/UR) principles when supporting referral workflows.
  • Provide consultation and administrative support for TRICARE beneficiaries.
