Lifeworks is hiring a full time Case Manager to work with Individuals with ID/DD. We are a nonprofit human service agency that provides adults with developmental disabilities the opportunity to live engaged and meaningful lives. We're searching for an enthusiastic individual who will bring their personality, an independent work ethic, and a strong belief in enhancing the lives of those we serve .
Essential Duties:
  • Assists individuals served within the program obtain and maintain employment that is consistent with their employment goals.
  • Collaborating with an exceptional team to plan and engage in the advocacy and support of our individuals, attending regular meetings as necessary.
  • Supporting individual’s self-employment, education, social growth, health wellness, communication skills, community mobility, and volunteerism.
  • Provide support through case management (electronic (Icentrix) and physical case records).
  • Writing and implementing Individual Support Plans (ISP) and completing all documentation.
  • Monthly and weekly progress notes for individual goals and objectives. Maintaining consistent communication with individuals and members of the ISP team.
  • Provide on and off-site support for Life Enhancement Curriculum
  • Assisting individuals served in areas of employment, social, creative, and health amp; wellness development and training.
  • Leading individuals served in community volunteer and learning experiences. Daily completion of payroll and billing documents.
  • Other duties and/or assignments at our different locations based on the needs of the team and the individuals you serve.
Requirements:
  • Experience working with Individuals with ID/DD
  • A Bachelor's degree or an associate degree in a related field with two years of experience, or HS diploma and five years of related work experience in Human Services.
  • Must be able to pass Fingerprint background check, CORI, DPPC and Driving background checks
  • Valid Driver's License required
  • Valid MAP certification is preferred
  • Excellent verbal and written communication skills
