Job description

Join our growing healthcareteam providing patient-centered primary care to residents in skilled nursing facilities throughout Mishawaka, Indiana. This part‑time position offers a flexible schedule, no call or overnight responsibilities, and the opportunity to work collaboratively with physicians and facility staff to deliver high‑quality, evidence‑based care.

Additional Opportunities Nationwide: Georgia, Illinois, Kansas, Michigan, Missouri, Ohio, Tennessee, and Wisconsin.

Position Highlights
  • Part‑time with flexible scheduling
  • Six‑hour facility shifts; rounds typically begin between 7:00 a.m. and 10:00 a.m.
  • No call or overnight shifts
  • Average 15–20 patient visits per day
  • Conduct daily rounds, manage acute and chronic conditions, prescribe medications, and develop treatment plans
  • Collaborate with nursing staff and interdisciplinary teams
  • Ensure compliance with clinical and regulatory standards
  • Company‑provided iPad with LTE

Requirements:
  • Active, unrestricted U.S. Nurse Practitioner or Physician Assistant license
  • Board Certification, Nurse Practitioners: ANCC, AANPCB, PNCB, or AACN. Physician Assistants: NCCPA
  • Board Certified or Board Eligible: FM, IM, Geri, PM&R & ED
  • Excellent clinical judgment, communication, and patient care skills
  • Ability to work independently while collaborating effectively within a multidisciplinary team
