Job description

Joining our Lake Regional Health System team means working alongside our knowledgeable, dedicated team of people who care for our patients and one another.
Lake Regional is seeking a dedicated, professional Registered Nurse (RN) Case Manager or Social Worker to join our team to join our team. This person will be tasked with helping us maintain a successful program and continue to foster our strong community relationships.
An annual influenza vaccination is required.
Accountabilities and Duties include but are not limited to
Develop a treatment plan responsive to patient/family concerns, age-specific problems and clinical impressions. Improves patient and/or family understanding of and adjustment to medical diagnosis through education.
Concisely documents needs that are identified, services provided, and discharge plan in patients' medical records.
Attends appropriate in-service, seminars, workshops, and training sessions related to Case Management to maintain and update clinical skills.
Supports hospital's philosophy and mission Formulates and maintains department policies and procedures.
Maintains confidentiality and complies with professional/ethical codes, state and federal regulations, and TJC requirements.
Facilitates multidisciplinary care plan meetings (rounding); recognizes, monitors, and participates in discharge planning needs; works cooperatively with multidisciplinary teams and outside providers and services to assure a smooth transition between levels of care.
Communicates with physicians, health plans, and other hospital staff regarding review outcomes, criteria, and various program requirements.
